Instructions
All coaches, assistant coaches, or anyone in any way involved with a team will need to fill out a CORI request form. It takes 2 minutes to complete. Please download the form, fill it out, and either give to a Board member, or email WJBLsecretary@gmail.com for instructions on how to turn it in.
